Haitian Creole Testing Application Application

NOTE:  If you have already started your testing in English (taken the knowledge and/or the skills exams), you may not apply to take your exam in Haitian Creole.

You are eligible for Haitian Creole testing if:
  • You have completed a Massachusetts Nurse Aide Training Program
  • IMPORTANT! You must proceed with the exam (both knowledge and skills exams) in the language you are registered for. If your application is approved, you must proceed with the exam in Haitian Creole.  You will not be able to switch languages, such as back to English.
INSTRUCTIONS: 
  1. Complete and submit this Haitian Creole Testing Application.
  2. Once approved, you will be emailed your Username and Temporary Password to sign in to your TMU© account.
  3. Please refer to the Massachusetts Nurse Aide Candidate Handbook for instructions on Completing your Account and other important testing policies and updates.
Once you have completed all the fields in this application, select 'Send Application' to submit it.
